Before Your First Visit

Please contact your insurance provider with the following info to confirm coverage:

  • Our clinic name: Wisteria Pain and Spine

  • Your plan’s name and/or network (sometimes the same insurer has multiple networks/tiers)

  • Whether we are in‑network for your specific plan

  • What your out‑of‑pocket cost will be (co‑pay, deductible, co‑insurance)

  • Plan to bring your insurance card and ID the first time you visit Wisteria Pain and Spine for care or if your insurance information changes.

    If you have coverage from more than one insurance company, we will bill the second insurance provider after the first one has paid. If you have health insurance coverage we’re not aware of, call and let us know so we can bill your insurance directly.

  • Depending on your insurance coverage, you may need to pay your co-payment (copay), deductible, or estimated patient portion during your visit. Contact your insurance provider or review your insurance card to learn more.

  • Your insurance provider will send you an explanation of medical benefits (EOMB or EOB). It will tell you the amount your insurance paid on a claim or the reason why all or part of the claim was not paid. Once your insurance has paid its portion, you’ll receive a bill for any remaining amount due.

  • If your insurance doesn’t cover the full cost of your care and you need assistance, you may:

    • Set up a monthly repayment plan. As you make payments, expect us to send you information regularly about the status of your account, including the amount currently due

  • Contact your insurance provider to find out whether your insurance requires pre-certification or authorization for any care at Wisteria Pain and Spine. If it’s required and you or your doctors don’t receive authorization, your insurance company may deny the claim or not provide full benefits.
